A few things to know about the intake process:

—you will not be contacted unless you complete the prospective patient form and sign it.

—You need to complete the intake packet 48 hrs before the initial appointment or risk rescheduling.

—Intakes are 90 minutes.

—If you late cancel or no show your intake appointment, you may not be offered an alternative time unless an exception is made by our office.
